The first appointment with your naturopath will consist of a thorough intake and physical examination, approximately 60 minutes, in order to develop and individualize your treatment plan. You will be asked to fill out a comprehensive intake form (click on the appropriate link below to download a copy that you may complete in advance): Adult Intake Form Pediatric Intake Form Please bring the following with you to your first appointment: Any current supplements and medications you are using, as well as any laboratory results you have available to you. The first appointment will end with a preliminary treatment plan and a planned follow up appointment. Follow up appointments typically last 30 minutes and include a review of your case.
